In other words, legitimate pain from various illnesses and somatic complaints can be intensified by the kind of messages we tell ourselves. Spontaneous self-defeating thoughts such as, “What’s the use, my body will always betray me and never get better.” can reinforce the pain cycle of making things worse. People with such chronic self-defeating reactions have been shown to create inner chemical changes and constricted blood flow which further erodes the individual’s ability to manage pain. How we respond to our bodily disorders, in terms of core beliefs and inner dialogue, may affect the outcome of our health.
Some time ago, I attended a presentation by psychiatrist M. Scott Peck. He talked with mental health providers about his struggles with neck pain, a problem that had plagued him for years. An operation resolved some of his pain, but he felt that there might be some negative underlying belief that was also contributing to the problem. He ultimately concluded that he was a conflict-avoider, lacking the ability to appropriately assert himself, refusing to “stick his neck out.”
Physical illnesses can be intensified by self-defeating underlying thinking that is a metaphor for the chronic condition experienced. For example, people with back pain may at times lack the “backbone” to express their thoughts and feelings courageously. Individuals with gastrointestinal problems may not be unable to “stomach” certain intolerable thoughts and feelings. People with headache syndromes may experience beliefs and thoughts about events that make them want to say, “Life is making my head hurt.” Eating disordered people may experience core assumptions such as, “I’m so angry that I could just vomit, or if I monitor my weight and eating habits, at least it’s one area in my life that I can control!” People with neurological pain such as inner ear disorders may exacerbate their pain by experiencing thoughts of panic such as, “Oh my God, here it comes again, that nasty, annoying pain. I’ll never get over this because the volume in my life is turned up too high.”
Anxiety, panic, and depression are typical characteristics associated with physical pain. The more effectively one manages these symptoms, the less troublesome the pain may be. Learning to cope with anticipatory anxiety by rationally responding (“Ok, I know that this pain can be troublesome, but when it comes I will do my deep breathing and manage just fine!”), or dealing with panic ( “When a wave of pain comes, I’ll just go with it. It’s not a big deal, my scary feeling are time-limited, they’ll be over soon”), and managing depression (“Just because I feel awful doesn’t mean I can’t do things to stay active and make me feel involved”) are important ways of adaptively responding to pain.
The following ideas are some guidelines for managing pain more effectively:
Try to get you pain in perspective. Make a realistic appraisal. “In the scheme of things, how bad is my condition?”
Don’t fight with your symptoms, it only makes them worse. The more you accept your symptoms, the more they are likely to diminish.
Use various activities to refocus away from your pain. Dwelling on pain makes it more painful. Stretching, music, swimming, meditation, and other activities are important.
Seek a multidisciplinary approach to your problem, if necessary. Get a team of healthcare specialists, including a quality physician, psychotherapist, physical therapist, message therapist or other providers of pain management.
Develop a solid support system of family and friends. Also, there are many support groups in our community for people suffering from a variety of physical ailments.
Remember, that the things we tell ourselves have an impact on our physical and emotional well-being.

Chronic Myofascial Pain & Fibromyalgia – Often Together BUT Miles Apart
Many people with fibromyalgia also have chronic myofascial pain or CMP (formally known as Myofascial Pain Syndrome, MPS) and don’t even know it. It is often missed because it is easy to confuse the pain and it’s origins with that of FM. As a result, it is missed in the diagnosis. Both are connected to the musculoskeletal system, which makes up almost 50% of our body weight, but should not be confused as being the same. Understanding FM and CMP and what makes them tick, will empower you to help yourself. You will be able to figure out some of the contributing factors to your pain, where it originates and what makes it feel better. It will help you understand treatments and find the one(s) that work for you.
It was recently discovered that MPS is not actually a syndrome at all, but a neuromuscular disease. This is important news! The difference? Diseases have known causes and a well-understood process for producing symptoms. Myofascial pain due to trigger points is now considered a true disease, rather than a syndrome. Fibromyalgia is a syndrome like rheumatoid arthritis and lupus and has tender points (not to be confused with trigger points). Even with these differences, it is believed by many researchers that one can influence the other.
I have mentioned before that I have a few experts that I follow closely and call my “fibro heroes” because they helped me through their studies and publications to understand and validate my pain. Devin Starlanyl is one of those heroes. Actually, she was my first. I read a book she co-authored, Fibromyalgia and Chronic Myofascial Pain: A Survival Manual, and quickly followed that by her book, The Fibromyalgia Advocate: Getting the Support You Need to Cope with Fibromyalgia and Myofascial Pain – she is amazing! She is a sufferer herself as well as a doctor/researcher. She set about trying to give doctors perspective from the patient’s point of view and has created diagnostic guidelines, patient resources with an empathy you can’t get anywhere else. She is at the forefront of all the research, news and information & shares it with FM & CMP sufferers in a way we can understand. Finding out about her work, was the beginning of my understanding.
What are trigger points? Trigger points are subtle, but taut bands that constrict the muscle and cause tremendous pain. Trigger points can be in the fascia that surrounds the muscle or in the muscle itself. They are extremely sensitive to pressure at the site and also cause “referred” pain – meaning pain at another location of the body. The part of the muscle fiber that actually does the contracting is a miniscule component called a sarcomere which cannot be seen by the naked eye. Contraction occurs in a sarcomere when its two parts come together and interlock like fingers. Myofascial trigger points are confined in one area and are dying for oxygen, which causes a demand for energy. There is a chemical reaction in central nervous system which sensitizes nearby nerves. This triggers the motor, sensitivity and autonomic (not under voluntary control) reactions of the trigger points. Muscles with trigger points are in a perpetual state of energy crisis.
Trigger points cause headaches, neck and jaw pain (TMJ), low back pain, tennis elbow, and carpal tunnel syndrome. They can cause pain in the shoulder, wrist, hip, knee, and ankle and are often mistaken for arthritis, tendonitis, bursitis, or ligament injury.
Trigger points can also cause dizziness, earaches, sinusitis, nausea, heartburn, false heart pain, heart arrhythmia, genital pain, and numbness in the hands and feet. Some experts believe that fibromyalgia may sometimes start as a result of myofascial trigger points. In CMP trigger points, the pain is more intense with a radiating pattern along the same muscle or muscle group. There is usually some loss of the range of motion, as well as a substantial weakness in the muscles that have active trigger points. The pain decreases when the muscle is at rest, and the intensity of the pain increases as soon as the that muscle starts to contract.
Proof of myofascial trigger points has been produced by the use of electromyographic imaging (a device that converts the electrical activity associated with functioning skeletal muscle into a visual record or into sound and has been used to diagnose neuromuscular disorders and in biofeedback training). Researchers have also used ultrasounds of localized twitch responses of trigger points. They can even do biopsies of myofascial trigger points that show the contraction knots and rounded muscle fibers. In one of Devin Starlanyl’s articles, I read that, as a result of this, The Journal of Musculoskeletal Pain has stated that the trigger points involve the “nerve terminal and the postjunctional muscle fiber” which “identifies myofascial trigger points as a neuromuscular disease”. What causes trigger points? There are many factors that can be attributed to trigger points: poor posture, scoliosis, thyroid deficiency, estrogen deficiency, loss of flexibility, nerve root compression (pinched nerve), emotional stress/anxiety that leads to lack of sleep which can increase muscle tension, fatigue and pain threshold. Other factors that MAY cause or worsen CMP trigger points are: nutritional deficiencies, chronic infections, muscle imbalance, inactivity (static posture).
When I injured my left knee, which resulted in surgery, I developed a limp and without even thinking about it, continued to favor (protect) that knee long after surgery. As a result, my entire left side became weak and I experienced pain that radiated from my lower back all the way to my ankle. My lower back and hip would tighten up so much to compensate for the muscles in my leg, that I sometimes couldn’t even move. The pain felt like all my muscles from the waist down on that side had “seized”. It was so intense, I would have to freeze in that position until it eased. It still happens today, but the difference is, I know what to do about it.
After a car accident, my FM and CMP were inflamed to a point that I was sent to a therapist for continual treatment. I had been reading about Myofascial Release Therapy and was really wanting to try it. I was pleasantly surprised to find a myofacial release therapist that had practiced under John F. Barnes, President and Director of the Myofascial Release Treatment Centers and National Myofascial Release Seminars. Barnes developed the most incredible, pain-easing therapy that makes your fascia and muscles feel like they are melting like butter and gently releasing and unfolding your muscles! Of course, that is MY non-clinical way of describing it, BUT that is what it felt like to me.
Being a military family, we eventually had to move to another base and I immediately started searching for a therapist that knew and understood the John Barnes method. I saw several therapists (covered by my insurance) that claimed to know it, but I was incredibly disappointed and ended up hurting more after a session than when I went in. So, my recommendation is to make sure these people have actually studied under Barnes or were trained at one of his seminars. Ask for proof. Carryover Pain Relief From Interferential Therapy is Not New, Only the Equipment Is
Today there is a great deal of “Medical Buzz” about the onset of carryover pain relief, also known as residual pain relief, after using portable interferential equipment for treatment of chronic pain. This is not a new phenomenon but has been going on for more than half a century and quite successfully. The “newness” is the same clinical results are occurring but without having to go to a clinic or hospital.
Why?
Interferential therapy has been used by physical therapists, chiropractors, pain clinics, physicians since 1953 with the same carryover pain relief results. It has been the mainstay of those medical professionals in treating patients suffering from chronic pain diseases. Some of the best results have been obtained for those who have been unable to find adequate treatments and they are successfully treated by “alternate”, non drug methods and interferential treatments are the mainstay for those treatments. Many other pain conditions where the patient has been doped out on drugs or told “there is nothing else we can do”. Often the processes of rehabilitation, such as you see where one has suffered a “frozen shoulder”, are enhanced and healing increased by using interferential therapy in conjunction with doing the painful exercises. The interferential helps to overcome the pain created by the exercises and shortens the pain that follows after the exercises are over. None of this is new though.
Let me describe the process that has been going on and on for decades and is totally dependent upon interferential providing carryover pain relief.
1. The patient enters the Chiropractic, Physical Therapy, Pain Clinic or Physician office. Generally at the time of entry the patient has been very uncomfortable due to pain. It has reached the point the patient needs help or it has reached the point the patient is tired of using drugs and not being able to live life due to the masking in the brain. The patient is not going to keep using the drugs but is now hurting and it’s to the point of being unbearable.
The health care practitioner will normally greet the patient and begin to either, if chiropractic, do a manipulation if indicated, or if Physical Therapy, apply warm moist heat and soft tissue massage or stretching. Most chronic pain patients have already tried surgery, injections, pills, and conventional treatments to no avail. They are looking for something that just might work.
After the practitioner has completed the initial treatment the chances are excellent the patient will then lay on a treatment table and begin a 12 to 20 minute interferential treatment. There is a large clinical machine beside the table and the practitioner will apply 2 to 4 self adhering electrodes to the patient. The machine is turned on with the instructions to the patient to tell the practitioner when the patient “feels the sensation”. The interferential machine is turned up till the patient declares it is feeling good to the patient and the pain has diminished. At this point the practitioner sets the timer on the interferential unit and will leave the room. The patient quietly lies on the table until the interferential machine stops treating. Once the machine stops then the patient in almost all cases actually feels better and the pain is either gone or severely diminished.
2. Now the “guessing game” begins. The patient, especially in the Chiropractic field, goes to the payment window and is asked to pay the bill. This is a crucial step in determining the efficacy of the interferential treatment. For many Chiropractic Physicians the service may not have been covered by insurance and the patient is paying out of pocket for the treatment.
If the treatment did not help the patient the patient will be:
1. Reluctant to pay the bill or,
2. Not come back for any future treatments
This is a critical moment for the patient and practitioner. In most cases due to the chronicity of the pain there will be follow up visits for care. The next appointment is also a guessing game. The guess is “when will the pain return” and a guess is made to have a return visit prior to the onset of pain, prevention is always the best course of action, or when will the patient be in so much pain an interferential treatment is needed. This isn’t something to ignore.
It will help to understand the patient now feels much better but the key is how long will the treatment last and the pain stay away. That is the carryover or residual pain relief period that interferential is famous for. That is also the guessing part of when to reschedule the return. This protocol is based on carryover pain relief and has been such for the last 60+ years clinically. It is also the proof of interferential therapy efficacy. The pain free period has to last at least till the patient pays the bill, and hopefully much longer so the patient will return!!!
It’s not only science but also patient success that matters. Did the interferential treatment work? If so the patient returns, if not,then good by or the next appointment is missed or not scheduled at all. That is the strength of interferential therapy over the decades. It has worked and worked well on the most difficult pain patients. The newness now is not that there is carryover pain relief, but with a patient being able to treat when needed with a portable interferential pain machine, then the carryover pain relief is getting extended over time. The newness of extended pain relief is what is and has emerged in the past 18 months.
It’s the advent of self treatment with interferential, without drugs, that is now challenging the understanding of the medical community.
Is there a physiological answer as to why longer periods of no, or reduced pain, is occurring?
It’s my contention there is.
As stated there is no surprise that interferential provides immediate and long lasting pain relief. What is surprising is how, with self treatments, the pain is prevented or treated and the treatments become less frequent.
There is an answer to the physiological side of electrical charges being used to heal. Nothing new as it’s well known anytime positive and negative charges of electricity are applied to the body there are very distinct chemical changes occurring. These charges have been used beneficially therapeutically for decades.
For years positive and negative charges have been used to promote bone healing, non union fractures, so the chemical and physiological changes are known. For many patients suffering with crush injuries amputation was the normal course until it was learned to electrically stimulate for unionization. The use of electricity would restart the process of the bones merging together and effecting a functionally stable reunion.
Equally impressive is the continuing use of electricity for healing bed sores, decubitus ulcers, which started with the use of the form of electricity known as pulsed galvanic stimulation. In the physical therapy profession small, portable electrical devices have been used to make bed sores fill in with tissue and eventually to close and heal completely.
Of course with bones and soft tissue repair the body must have the necessary nutrients to effect the changes electricity can do. Electrical charges alone will not suffice since the electricity initiates the body’s response to heal and the body requires the necessary nutrients and hydration to accomplish the task.
There is no doubt with the pain patient that there exists a chemical process that is causing the pain sensation to be created and transmitted to the brain. That process is indicative of sickness, since chronic pain of unsubstantiated cause, is unnatural and requires therapeutic healing to stop the injurious processes. Possibly the use of interferential with the positive and negative charges is chemically altering the pain stimulus area and the repeated usage of the interferential unit to stop and prevent pain, is altering the chemistry of the area. That alteration may become permanent and it is that permanency which is stopping the unnatural pain impulses. With preventive treatment the body is stopping the chemical changes before they can cause pain.
The electrical intervention is supporting the permanent change that we call carryover pain relief.

It also produces blood vessel constriction in localized tissues to decrease swelling and relax skeletal muscles. As a general rule, cold therapy is indicated during acute inflammation (up to 72 hours after injury or surgery) when the area shows signs of redness, swelling, pain, and or heat. The treatments should last 15 to 20 minutes and should be performed three to four times a day for the first 72 hours following the injury or surgery. Be sure to check the skin several times during the treatment to prevent the skin from getting too cold.
After the inflammation has subsided, the application of heat is beneficial. Heat applied to the affected area causes mild muscular pain relief, sedation, and increased local circulation. An easy way to apply heat to the affected area is through a warm hot water bottle or moist washcloth heated in the microwave. (the temperature of the heat source should ideally be between 104 and 113 degrees. The warm pack treatment should last 15-20 minutes and should be applied three to four times a day. It is important to check the skin every few minutes during the treatment. If the pet’s skin is hot to the touch, use more towels to insulate the heat source.
